Chase Nielsen, DO, is a board-certified primary care physician who has a passion for helping others with such a significant part of their lives - their health. Dr. Nielsen values the relationships he establishes with his patients and places emphasis on being a resource to help them during life's difficult and joyous moments. Providing care at PMC - Sanford on Carthage Street, Dr. Nielsen offers services such as annual physical exams, disease prevention and health maintenance, pediatric patient visits, sports physicals, and minor surgical procedures.
